CONDITIONS · LOW AMH AND LOW OVARIAN RESERVE

Your AMH is low. It is a count, not a verdict.

AMH estimates how many eggs are left, not how good they are. A low result changes how we plan treatment and how quickly we move, not whether you can have a baby.

  • Measures egg numbers, not quality
  • Read with your age and a scan
  • Natural pregnancy still possible
  • Time matters: act, don't wait

01

What does a low AMH level mean?

A low AMH means fewer eggs are left in the ovaries than usual for your age, so fewer eggs are likely to grow in one IVF cycle.

AMH (anti-Müllerian hormone) is made by the small follicles that hold eggs. It can be tested on any day of the cycle, and it is read together with an antral follicle count on a scan and your age. Labs use different units and ranges, so compare your result with your age, not with a friend's. If one result does not fit the rest of the picture, it can be repeated.

02

Can you get pregnant with low AMH?

Yes. Natural pregnancy needs one good egg a month, and women with low AMH still release an egg in most cycles.

AMH predicts how many eggs IVF will collect. It is a poor predictor of whether you will conceive naturally. Many women with low AMH conceive without help, particularly when they are younger and the other tests are normal.

Honest note

Low AMH predicts egg numbers, not whether an egg is healthy. On its own it only weakly predicts pregnancy, and it should not be used to turn someone away from treatment. (What the evidence says (ASRM, 2020))

03

Why does age matter more than the AMH number?

Egg quality follows age, so a low AMH at 30 and a low AMH at 42 mean very different things.

At 30, a low AMH usually means fewer eggs of reasonable quality. After 40, fewer eggs are also more likely to carry chromosome errors, which is what lowers the chance of pregnancy and raises the chance of miscarriage. Ovarian surgery, chemotherapy and smoking can also lower AMH at any age.

04

What are the treatment options for low AMH?

The options are a natural try for an agreed time, IVF planned to collect and bank as many eggs or embryos as possible, egg freezing if you are not ready yet, and donor eggs.

Higher doses of fertility drugs cannot create follicles that are not there. What helps is stimulation fitted to you, and collecting eggs over more than one round, sometimes twice in one cycle (dual stimulation). Donor eggs are an option when your own eggs are unlikely to work. In India they come only through a registered ART bank, and the donor's identity is kept confidential under the ART (Regulation) Act, 2021.

Honest note

Egg accumulation and individualised stimulation, including dual stimulation, rather than simply higher doses. (Dr Vani Sundarapandian, on how she treats low ovarian reserve)

How much time do you have?

Nobody can give you a date, but a low AMH means the reserve will keep falling, so the right move is to act now rather than wait.

Get both of you tested, agree a plan and a time limit for each step, and don't lose months to treatments that cannot help. If your reserve is extremely low and you are over 40, Dr Vani will say plainly if treatment with your own eggs is unlikely to help, and talk you through donor eggs. If you are not ready for a baby yet, egg freezing can keep today's eggs for later.

Does this sound like you?

Do any of these sound like you?

  • My AMH report says 'low' and I don't know what it means
  • Few follicles seen on my scan
  • Few eggs collected in a past IVF cycle
  • I've had surgery on an ovary
  • I'm over 35 and want to know how much time I have
  • I'm not ready for a baby yet, but I'm worried

Not tested yet? Start with what an AMH test can and can't tell you.

The order we usually work in.

  1. 1

    Check the whole picture

AMH with a follicle-count scan, your age, a tube check, and his semen analysis.

  2. 2

Natural try, with a time limit

If you are young and everything else is normal, trying naturally for a short, agreed time is reasonable.

  3. 3

    IVF, planned for low reserve

Stimulation fitted to you, eggs or embryos banked over more than one round, sometimes with dual stimulation.

  4. 4

    Donor eggs

When your own eggs are very unlikely to work. Discussed honestly, never pushed.

  5. Not ready yet

    Egg freezing

Keeps today's eggs for later. The younger you freeze, the better the eggs.

Questions patients ask

Still unsure? Ask us on WhatsApp — a real person replies.

Can I get pregnant naturally with low AMH?

Yes, many women with low AMH conceive naturally, because you still release an egg in most cycles. Your age and your other test results matter more for natural pregnancy than the AMH number.

Does low AMH mean my eggs are poor quality?

No. AMH measures how many eggs are left, not how healthy they are. Egg quality depends mainly on age.

Can AMH be increased?

No proven medicine, supplement or diet raises AMH or adds eggs. Stopping smoking protects the eggs you have, and time is better spent on a plan than on supplements.

Is IVF possible with low AMH?

Yes, IVF works with low AMH, though fewer eggs are collected each round. Stimulation is planned for low reserve, and eggs or embryos can be banked over more than one collection.

Does low AMH mean early menopause?

Not necessarily. A very low AMH suggests menopause may come earlier than average, but it cannot predict when. Periods that stop before 40 need a separate check.

Should I freeze my eggs if my AMH is low?

If you are not ready for a baby yet, freezing eggs sooner rather than later is worth discussing. With low AMH, fewer eggs are collected each round, so it may take more than one.

In their words, not ours.
