Beta hCG is a blood test that measures how much pregnancy hormone is in your blood. After fertility treatment it answers two questions: has an embryo implanted, and is the level rising as it should?

What does a beta hCG test measure?

A beta hCG test measures the exact amount of hCG, the hormone made by the cells that will form the placenta, in your blood.

hCG appears within days of an embryo implanting and rises fast in the first weeks. A home urine strip only says yes or no above its threshold. The blood test gives a number (in mIU/mL) that can be repeated and compared, and the change between two numbers carries far more information than either one.

When should you test after an embryo transfer or IUI?

Test on the date your clinic gives you, and not before: at Jananam that is usually day 12 after a blastocyst transfer and day 14 after a day-3 transfer or an IUI [CONFIRM].

Implantation takes a few days, and hCG takes a little longer to become measurable. A test taken too early can be negative in a pregnancy that is doing well, or positive only because of the trigger injection. The waiting is hard. The date is not arbitrary.

Why does the second result matter more than the first?

Because the range of first results in healthy pregnancies is very wide, what tells you more is how the level changes over 48 hours.

Two people can start at very different numbers and both have an uncomplicated pregnancy. There is no first number above which you are safe and below which you are not. NICE uses a rise of more than 63% in 48 hours as a sign of a developing pregnancy in the womb, still to be confirmed by scan. A slower rise, a flat level or a fall needs a review within a day, not waiting at home.

What can a beta hCG test not tell you?

A beta hCG cannot tell you where the pregnancy is, whether it is twins, or how it will go; only a scan can show where it is and how many.

A high number is not proof of twins, and a low one is not proof of loss. The trigger injection contains hCG and can stay in the blood for about 10 to 14 days, so an early home test may show the injection, not a pregnancy. A pregnancy is usually too small to see on a scan until about 5 to 6 weeks, which is why blood tests come first.

Can an ovulation test detect pregnancy?

An ovulation (LH) test can sometimes turn positive in early pregnancy, but it is not a reliable pregnancy test and should not be used as one.

LH, which triggers ovulation, and hCG are built alike, so some ovulation strips react to high hCG. A pregnancy test is made to detect hCG and turns positive earlier. After fertility treatment, the blood test on your given date is the one that answers the question.

Units: mIU/mL and IU/L are the same value (1 mIU/mL = 1 IU/L), so a report in either unit reads the same.

What is a good beta hCG level 14 days after embryo transfer?

There is no single good beta hCG level at 14 days, because healthy pregnancies start across a very wide range. What matters is that the result is clearly positive and then rises as expected over the next 48 hours.

How fast should beta hCG rise?

In early pregnancy, beta hCG usually rises quickly, and NICE treats a rise of more than 63% over 48 hours as a sign of a developing pregnancy in the womb. The rise slows naturally as the level gets higher, and by then a scan is more useful than more blood tests.

Does a low first beta hCG mean the pregnancy will fail?

No, a low first beta hCG on its own does not mean the pregnancy will fail. It can reflect later implantation or normal variation; the repeat test 48 hours later tells you much more.

Does a high beta hCG mean twins?

Not reliably. Twin pregnancies tend to have higher levels, but the ranges overlap so much that only a scan can tell.

Can the trigger injection cause a false positive pregnancy test?

Yes, an hCG trigger injection can cause a false positive, because it is the same hormone the test detects and takes about 10 to 14 days to clear. That is why we ask you not to test early at home.

When should I take a pregnancy test after a frozen embryo transfer?

Take it on the date your clinic gives you: usually day 12 after a frozen blastocyst transfer, or day 14 after a day-3 embryo. A blood test on that day is more reliable than any home test before it.

Can an ovulation test detect pregnancy?

Sometimes an ovulation test turns positive in early pregnancy, because LH and hCG are similar, but it is not reliable. Use a pregnancy test, or after treatment, the blood test on your given date.

What if my beta hCG is between 5 and 25?

A result between 5 and 25 is borderline, so the next step is a repeat test in 48 hours, not a decision. It can be a very early pregnancy, leftover trigger hormone, or a pregnancy that is not continuing.
