IUI: a simple first step, with a limit agreed first.
IUI (intrauterine insemination) places washed sperm inside the womb, close to the egg, at the moment of ovulation. It is simpler than IVF, and for the right couple it is a sensible place to start.

01What is IUI treatment?
IUI treatment places washed, concentrated sperm inside the womb at the time of ovulation, so more moving sperm start closer to the egg.
The semen sample is washed in the lab to remove the fluid and keep the moving sperm. A soft, fine catheter passes through the cervix and releases them into the womb. Fertilisation still has to happen inside the fallopian tube, on its own. That is the main difference from IVF, where eggs and sperm meet in the lab.
02Who is IUI for, and who should skip it?
IUI suits couples with unexplained infertility, a mildly low sperm count, ovulation problems such as PCOS, mild endometriosis, difficulty with intercourse, or donor sperm.
Three things must be true first: you ovulate (or tablets can make you), at least one tube is open, and the washed sperm sample is good enough. If both tubes are blocked, the sperm count is very low, endometriosis is severe or egg reserve is low, IUI mostly costs time. IVF is then the shorter road.
03How is the IUI procedure done?
The IUI procedure itself takes a few minutes: a speculum, as in a smear test, then a fine catheter passes the washed sperm into the womb.
Your partner gives a sample that morning and the lab prepares it, which takes about an hour. You lie on a couch; there is no anaesthesia and no cutting. Most women feel it as uncomfortable rather than painful, with some cramping. You can get up and go home straight after.
04What decides the success rate of IUI?
Your chance with IUI depends mostly on the woman's age and egg reserve, whether the tubes are open, how many moving sperm are left after washing, and how long you have been trying.
Well-timed cycles with a mature follicle matter too. A clinic-wide percentage tells you little, because it depends on which couples that clinic treats. At the first consultation we go through your own age, tests and history and tell you what IUI can realistically do for you, including when it is unlikely to help.
05How many IUI cycles should we try before IVF?
At Jananam the number of IUI cycles is agreed by age before you start: fewer as age rises, and IVF sooner if egg reserve is low.
If IUI is going to work for a couple, it usually does so in the first few well-timed cycles. Many couples come to us after a year of repeated IUI because no one set a limit. Cycles are done back to back, and the plan changes early if a tube turns out blocked or the sperm count falls.
Under 35 with good reserve: 3 to 4 IUI cycles. 35 to 37: 2 to 3. 38 and over: 1 to 2 if reserve is satisfactory. Sooner to IVF if reserve is low or endometriosis is advanced. (Dr Vani Sundarapandian, Medical Director)
06What happens after IUI?
You can go back to normal life after IUI; bed rest does not improve the chance of pregnancy.

Scans measure the follicle and the lining. They decide the timing, not the calendar. Too many follicles means the cycle is stopped, to avoid triplets or more.

What is the success rate of IUI?
The chance of pregnancy with IUI depends on the woman's age and egg reserve, open tubes, the washed sperm count and how long you have tried, so one clinic-wide number would mislead you. At your first consultation we look at your own tests and tell you plainly what IUI can do for you.
Does the IUI success rate change with age?
Yes, the chance with IUI falls as the woman's age rises, mainly because egg numbers and quality fall. That is why Dr Vani offers fewer IUI cycles from 35, and only 1 to 2 from 38, before moving to IVF.
Is the IUI procedure painful?
IUI is usually uncomfortable rather than painful, similar to a smear test, with some cramping during and shortly after. No anaesthesia is needed, and light spotting for a day is normal.
How long does IUI treatment take?
One IUI cycle takes about two to three weeks from your period to the insemination, and the insemination itself takes a few minutes. The pregnancy test follows about 14 days later.
Does IUI work if a fallopian tube is blocked?
IUI needs at least one open tube, because fertilisation still happens inside the tube. If both tubes are blocked, IUI cannot work and IVF is the right treatment.
What is the difference between IUI and IVF?
In IUI, washed sperm are placed in the womb and fertilisation happens inside the body; in IVF, eggs are collected and fertilised in the lab, and an embryo is placed in the womb. IUI is simpler; IVF can work where IUI cannot, such as blocked tubes or a very low sperm count.
Can IUI be done with donor sperm?
