Most couples who reach her have been told to relax, or to try one more cycle. She reads your file first, tells you what will help and what won't, and agrees a stopping point with you before treatment starts.

WHY SHE STARTED JANANAM
Jayalakshmi, her mother's elder sister, married in the 1940s and never had a child. Treatment then was largely inadequate and childlessness was treated as a disgrace, not a medical condition. She died of blood cancer at 34, before Dr Vani was born, and Jananam was founded in her memory.

We do not transfer more than two embryos under any circumstance. Single embryo transfer is preferred for tested euploid embryos at any age, and usually under 38 with a good blastocyst.
More than 90% of our embryo transfers are frozen. It lets the womb lining recover from stimulation and is needed when embryos are tested.
We have used ERA in more than 750 patients in the past, but in our experience it did not improve live birth rates sufficiently for us to continue recommending it.
Under 35 with good reserve: 3 to 4 IUI cycles. 35 to 37: 2 to 3. 38 and over: 1 to 2 if reserve is satisfactory. Sooner to IVF if reserve is low or endometriosis is advanced.
ICSI should not simply be viewed as automatically better IVF. We use it where it is indicated, not by default.
Egg accumulation and individualised stimulation, including dual stimulation, rather than simply higher doses.
Couples wait too long to resume treatment after a failed cycle.
We agree the stopping point with you before your first cycle. The usual reasons are extremely low reserve over 40, repeatedly poor eggs, failed fertilisation, consistently poor embryos, repeated embryo arrest or no viable sperm.

Planning around fewer eggs: collecting over more than one cycle instead of pushing doses higher.
Good embryos that have not implanted, more than once. She looks for a cause before suggesting another transfer.
Repeated miscarriage, investigated for both partners, with tests only where they change the plan.
Plain answers to what couples search at 2am, reviewed before publishing.

Profile
She qualified in medicine (MBBS) at the University of Madras and went on to an MD in Obstetrics and Gynaecology and a DGO. She practised in the United Kingdom and is a Fellow of the Royal College of Obstetricians and Gynaecologists (FRCOG). She also holds a Master's in Reproductive Medicine from the University of New South Wales, Australia. She is a member of ISAR, ESHRE and FOGSI.
She returned to India and founded Jananam in 2013, in memory of her Periamma, Jayalakshmi, who never had the child she wanted. Jananam treats fertility only. Its main hospital and IVF lab are in Neelankarai, off East Coast Road, with a second clinic in Thiruporur led by Dr Mangalam Raman.
Her special interests are low ovarian reserve, recurrent implantation failure and recurrent pregnancy loss. Many of the couples she sees have already had a failed cycle elsewhere. She reads each couple's history before the first consultation and builds the plan around it. Both partners are checked before any plan is made. Tests are ordered when the result would change what happens next.
Her positions are written down and published. She does not transfer more than two embryos. Most transfers at Jananam are frozen. She used the ERA test in more than 750 patients, then stopped recommending it because, in her experience, it did not improve live birth rates enough. When the evidence for an add-on is weak, she says so.
Before a couple's first cycle, she agrees with them when she would change the plan, when she would pause, and when she would recommend stopping. The reasons are clinical and set out in advance, so the decision is never made in a hurry after a failed cycle.
Consultations are not hurried. Patients who travel from abroad usually start with a video consultation with her, and the same doctor stays with them through treatment and follow-up.
